New Patient Registration: Registration Form

The doctors welcome new patients who live within our practice area.

To register you must complete a registration form which is available from our reception area or can be downloaded:

During the registration process, and providing your entitlement to NHS care has been established, you will be asked to provide a current passport, or, if you do not have one, some other form of photographic evidence to confirm your identity, together with evidence to confirm your current address. Prior to completing a registration form we would ask that you check to ensure that you have at least a 2 week supply of any regular medication, from your existing doctor. This is because it takes time for your medical records to be received in our surgery. On completion of the registration form you will be given an appointment for a new patient health check with one of our Health Care Assistants or Practice Nurses and you may also be given a GP appointment for review of medication.  Failure to attend this new patient medical will result in your application to join the practice being refused.

Where patients are requesting to join the practice list, the practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.
